Türk hekimlerin yaşadığı etik ikilemler ve buna bağlı duygu durumları: Nitel bir analiz

Özet
Etik ikilem, karmaşık ve çözülmesi güç konulardan biridir. Özellikle yapısal olarak kompleks bir niteliğe sahip olan sağlık kurumlarında, hekimler çeşitli etik ikilemlerle karşı karşıya kalmakta ve bu durum onları zor kararlar almaya zorlamaktadır. Hekimlerin yaşadıkları etik ikilemler ahlâki sıkıntı yaratabilmektedir. Bu nedenle, bu çalışmanın amacı hekimlerin yaşadığı etik ikilemleri ortaya koymaktır. Çalışma kapsamında, açık uçlu yazılı anket aracılığıyla 55 hekime ulaşılmıştır. Elde edilen veriler tematik analiz ve betimsel analiz yöntemiyle incelenmiş, analiz sürecinde NVIVO 14 Plus paket programından yararlanılmıştır. Çalışmanın bulgularına göre, hekimlerin tıbbi karar verme süreçlerinde etik, sistemsel ve kurumsal unsurlarla birlikte; mesleki kimlikleri ve kişisel değerleri arasında ikilemler yaşadıkları görülmüştür. Ayrıca hekimler, çoğu zaman tıp etiği ilkelerinin birbiriyle çatıştığı durumlarla karşılaşmaktadır. Bu etik ikilemler, hekimlerde çaresizlik, korku ve yetersizlik hissi gibi temel duygusal tepkilere neden olmakta; bu durum da mesleklerini icra ederken zorlanmalarına yol açmaktadır. Hekimlerin etik ikilemlerini tespit etmek, hasta haklarının korunması, hasta güvenliği, tıp eğitimi ve politika gelişimi gibi birçok noktaya katkı sağlamaktadır. Bu açıdan ikilemleri azaltmak için klinik karar destek sistemleri güçlendirilebilir.
Ethical dilemmas are complex and difficult to resolve. Especially in structurally complex healthcare institutions, physicians face various ethical dilemmas, forcing them to make difficult decisions. The ethical dilemmas experienced by physicians can create moral distress. Therefore, the purpose of this study is to reveal the ethical dilemmas experienced by physicians. Fifty-five physicians were contacted through an open-ended written survey. The data obtained were analyzed using thematic and descriptive analysis methods, and the NVIVO 14 Plus package program was utilized in the analysis process. The study findings indicate that physicians experience dilemmas between ethical, systemic, and institutional factors, as well as their professional identities and personal values, during medical decision-making processes. Furthermore, physicians often encounter situations where the principles of medical ethics conflict with each other. These ethical dilemmas evoke fundamental emotional reactions such as helplessness, fear, and feelings of inadequacy in physicians, which in turn lead to difficulties in practicing their profession. Identifying physicians' ethical dilemmas contributes to many aspects, including protecting patient rights, patient safety, medical education, and policy development. Clinical decision support systems can be strengthened to reduce these dilemmas.
